As a schizophrenic (schizopaffective depressive type), I understand when your sick mind makes you do horrible things. How I cope with it is doing the best I can to prepare when I'm healthy for when I'm sick. I spend hundreds of hours on this every year. It's kinda appalling to see this narcissist do zero things to better themselves. Also I have no idea why you shouldn't be able to discuss people's mental conditions. If a footballer breaks his leg people will know. It's the same, if it's obvious to most the leg is broken, we see the bone jutting out.
