The head armorer was under-qualified and allowed the crew to "target shoot" with the prop guns and real bullets. The crew left one of the guns loaded with real bullets. Then the armorer failed to check the guns/remove the real bullets. Then the assistant director failed to check the gun before announcing it was "cold" and handing it to Alec Baldwin; who then failed to check the gun before pointing it at someone, announcing his intent to fire the weapon (assuming it was properly loaded with blanks) and pulling the trigger.

Regarding that gun having an actual bullet in it, apparently, the armourer in charge for this kind of stuff was actually playing around with live ammunition using the gun and, when she placed the gun back, she left a bullet in.

They were playing around with live ammo between shots (pun intended), using the same guns that were in the film and a large part of the crew had gone on strike over safety concerns just hours before the deadly shooting happened. There had reportedly been at least 3 negligent discharges already as well.
